For birth and death certificates, please include the following in your request.

• Purpose of request
• Name at birth/death
  Date of birth/death
  City and county of birth/death
  signature of subject or guardian
  Address
  Telephone Number
For birth records also include:
• Father's name
• Date & place of birth
• Mother's name
  Date & place of birth
• U.S. Passport information

Kindly indicate the number of documents desired - additional copies $10 each. Check or money order must be signed, dated, and made payable to the Department of State. Remittance must be payable in U.S. dollars through a U.S. bank. Do not send cash.
